This assessment is a required part of our Single Session, 8 Week Sessions, On-T.A.P. Collective Group Coaching, and Membership Programs. Your honest completion of this assessment will help guarantee a positive outcome for our time together. The more detail you give the better. All comments provided are 100% confidential and will not be shared with anyone without your express permission.
As part of our personal integrity, transparency, and positive outcome commitment, we give MEN the option to coach with Jeffrey alone or with Jeffrey & Shanda together. And we give WOMEN the option of coaching with Shanda alone or with Shanda & Jeffrey together. Jeffrey doesn't coach women alone & Shanda doesn't coach men alone.

We welcome the opportunity to get to know more about you and you us. It's important that we determine if we're a good fit for one another. Your time and resources are valuable and so is ours. We require new coaching clients to complete an assessment to tell us a little about them and what they'd like to gain from coaching with us. You'll have the option to coach with either Jeffrey, Shanda, or both.
